This will serve as our initial contact. During this stage, we will emphatically hear and understand the care needs of your loved one. We will answer any questions you may have and explore care options to consider in this complex healthcare world.

We book a complimentary In-home care assessment.
We offer plenty of flexibility to accommodate your busy schedule, as such, a virtual option can be explored. During the initial in-person visit, our Care Manager will assess the unique client goals and needs while also allowing you or your family to know more about us. In addition we will also assess the environment to ensure a conducive and safe home for you and your loved one.

After compiling all the gathered information from the in-home care assessment then our competent care managers will develop an individualized care plan. This care plan will then be presented and reviewed by you and the rest of your family. Meanwhile, internally, the process of matchmaking and choosing the best caregiver for your loved one has started. Ensuring the right personality, qualification and interest are taken into account. The Journey Begins.
Our caregivers will arrive for their initial and succeeding visits. The case manager will render an ongoing assessment and continuous evaluation of the delivery of care. The care plan will be updated as the need arises, to ensure our team are meeting the client’s goals.
